Manager for Care Coordination

Position Overview:

VITALity Catholic Healthcare Services is seeking a self-motivated, flexible, and compassionate Registered Nurse with a background in community-based nursing to lead and support our Care Coordination program. The ideal candidate is a self-starter, flexible, and committed to improving care access and delivery for the elderly and disabled throughout the six counties of the Diocese of Camden.

Key Responsibilities:

  • Manage day-to-day operations of the Care Coordination team, including RNs, MSWs and care staff, across the Diocese.
  • Oversee intake and referral activities, including the Care Coordination Resource & Referral line.
  • Collaborate with the Executive Director on program development, housing wellness nurse oversight, and staff hiring.
  • Establish and nurture referral networks with local healthcare and social service providers.
  • Ensure delivery of person-centered care coordination services and promote continuous quality improvement.
  • Coordinate education and professional development opportunities for team members.
  • Support the promotion and expansion of the program through community presentations and parish engagement.
  • Collect and analyze service outcomes and manage program budgets responsibly.

Qualifications:

  • Bachelor’s degree in Nursing or a healthcare-related field (required).
  • Current RN license in New Jersey (required).
  • Minimum 5 years clinical experience, with at least 3-5 years in a leadership or supervisory role.
  • Strong background in care coordination, transitions of care, community health, and healthcare navigation.
  • Excellent interpersonal, communication, and organizational skills.
  • Experience in faith-based or parish settings is a plus.

Salary Range:  $60,000 to $70,000
