Health Care Navigator (SSVF)

SUMMARY:     The Health Care Navigator connects Veterans to VA health care benefits or community health care services if Veteran is not enrolled in or eligible for VA health care.  This position reports to the SSVF Regional Program Coordinator and works collaboratively with the SSVF Case Management Team.  The Health Care Navigator bears no supervisory responsibilities.

DUTIES AND RESPONSIBILITIES:

  1. SSVF Health Care Navigator provides case management, care coordination, and referrals to community resources
  2. Assists Veterans in accessing healthcare, whether gaining entry to VA health care (including mental health care) or community care when Veterans are not eligible for VHA
  3. Acts as a liaison between the SSVF Grantee and the VA or community medical clinic
  4. Works with veterans (and their families) to ensure transportation for medical appointments, prescription medications, and related needs.
  5. Communicates routinely with the SSVF Case Manager as a collaborative partner for supportive services
  6. Identify non-clinical barriers to Veteran’s ability to access health care services and clearly state limitations, risk factors and support services needed
  7. Veteran’s needs and preferences in health care will be identified and resources provided to support those needs and preferences
  8. SSVF Health Care Navigator educates Veteran (and family) regarding health care providers recommendations and encourages questions
  9. Documents Veteran’s progress and provides updates to Case Manager
  10. Complies with SSVF Program requirements to ongoing trainings
  11. Participates in Team Meetings, Case Conferencing and Program activities

QUALIFICATIONS:

  1. Candidate will adhere to ethical principles about confidentiality, informed consent, agency policies, critical incident reporting, HIPAA, and Duty to Warn.
  2. Candidate works independently and is excellent with time management.
  3. Commitment to the Mission of Catholic Charities
  4. Excellent organizational and time management skills
  5. Good oral communication skills
  6. Ability to problem solve and multi-task
  7. Handle sensitive contacts with clients
  8. Ability to work independently and in a team setting
  9. Adequate computer skills and ability to learn new software

EDUCATION OR EXPERIENCE:

  1. Bachelor’s degree or equivalent combination of education/experience
  2. The SSVF Health Care Navigator has at least two years of experience in a health care or social services area of practice.
  3. Valid driver’s license and auto insurance

WORK ENVIRONMENT:  This position works in the main office at 1845 Haddon Avenue Camden but requires travel to Catholic Charities Family and Community Centers in Cape May, Atlantic, Cumberland, Salem and Gloucester, Camden Counties and other locations.

This position is grant-funded and is contingent on renewal of the grant.

Pay Range $32,800 – $45,500
